Instructions — what the model was asked

This folder is a frozen copy of the Proof repository, taken at the March 17, 2026 collaboration outage. It is a git repository whose first commit is that frozen state. Its npm dependencies are already installed in node_modules/ and work offline: npm test, npx tsc and npx tsx run from here with no network. Do not try to install or fetch anything.

Before investigating, read AGENTS.md, AGENT_PROMPT.md, benchmark_context/issue-557.md and benchmark_context/local-only-rules.md in full. The task in AGENT_PROMPT.md is quoted below; the “How to work” instructions here extend it from a plan to an integrated implementation and verification. For implementation scope, these instructions supersede any plan-only wording and any repository guidance that requires retaining the legacy architecture. Where a general instruction conflicts, the Benchmark Fixture Override in AGENTS.md wins. Paths in those files that point to another machine mean this folder.

This is a blind, local-only benchmark. Use only the files in this folder and its git history. Do not use web search, external services or any other copy of Proof.

The task, in the owner's words:

The code in this repo is vibecoded slop and it just keeps going down, and there's tons and tons of unrelated issues that are cropping up where it goes down or documents get duplicated, and I'm just tearing my hair out on it. I have a feeling that it's just vibecoded slop. If we started from the beginning, the codebase, especially around the live document collaboration, we would structure it way differently.

So if we wanted to do a clean first-principle structural rewrite where we were not thinking about, okay, what are the implementation services that we keep consistent? How do we do a clean migration? We just started from the beginning as a clean concept. What would we do? How would we structure it? What are the invariants that we would hold to be true throughout the codebase? Make a plan for that.

How to work:

  1. If you have to choose between a greenfield and a brownfield approach, choose GREENFIELD. Greenfield means a clean architecture for the working app, not a standalone prototype. You do not need a production migration or gradual rollout.
  2. Write your plan first and save it in docs/plans/.
  3. Then implement the plan in this folder. The default build and npm run serve must use the new core through the existing editor and APIs. Preserve the app’s intended capabilities, fix the incident failures, and route normal reads and writes through the new core without falling back to the legacy implementation. Actually launch the integrated app and exercise it yourself locally, and write and run automated integration/end-to-end tests against the running client/server paths, including multiple clients, persistence across restart, and the incident regressions or their mapped equivalents wherever the fixture permits. Use a browser if one is available; otherwise exercise the client/server API and collaboration protocol paths with local clients. Writing tests without running them is not verification. Retire an existing test only by naming it and what now covers it, including the regression tests written after past incidents; tests for anything you did not replace stay and still pass. npm run build, npm run test:ci and npm run test:pr:collab pass today and must pass at the end. Do not change .github/workflows; if a script runs a test you retired, update that script and say why in ANSWER.md.
  4. Before you stop, write ANSWER.md in the root of this folder. This file is required: it is your final report, and a chat reply alone is not read. It must contain:
    • the scope you delivered, and where the plan and the new code live
    • every build, typecheck and test command you ran, with its result
    • a capability inventory showing what was kept, changed or is missing, and the regression coverage
    • what you directly observed in the running app versus what remains untested, including any fixture or runtime limitations
    • the limitations and gaps that remain

About this sandbox: the internet is blocked, but you can start servers and connect to them on 127.0.0.1. $PORT holds a free port for this run, and Proof's server uses it. There is no writable /tmp; use $TMPDIR instead, for example APPSIGNAL_LOG_PATH=$TMPDIR npm run serve. This is one non-interactive session: it ends when you end your turn, and anything still running in the background is stopped then, so wait for background work to finish before you end your turn.

Delivered: an integrated, greenfield document authority

The default npm run serve now boots server/core/main.ts through server/bootstrap.ts. It serves the existing built Milkdown editor and existing document/agent API URLs using the new core. There is no default-runtime fallback to server/index.ts, server/collab.ts, server/db.ts, the legacy document engine, or their repair/admission services. Those files remain as historical regression fixtures and operator-source context.

The plan was written before implementation in docs/plans/2026-10-05-document-core-rewrite.md. The new implementation lives in server/core/, with the running integration suite in src/tests/document-core-integration.test.ts. Client integration changes are in src/bridge/collab-client.ts and src/bridge/share-client.ts; shared schema/format helpers received type corrections and the image parser fix. preview.json describes the server preview on this session's port, 57669. Dependencies were already installed; no installation, fetch, web research, deployment, or other repository copy was used. The baseline comparison came from this folder's first commit via local git archive.

Run from this folder:

APPSIGNAL_LOG_PATH=$TMPDIR npm run serve

The server uses $PORT, or 4000 if it is absent. New persistent data defaults to data/document-core.sqlite; set PROOF_CORE_DB_PATH to another local path to isolate a run. This is a new database/schema, with no production migration. Existing frozen legacy databases are not imported. data/ and .tmp/ are ignored local runtime artifacts.

What changed and why

The frozen incident report identifies multi-document stale-store/admission thrash as its strongest hypothesis, not a proven root cause. The fixture cannot establish the precise production initiating event. The implementation removes that mechanism structurally:

  • One authority: the Yjs ProseMirror fragment and marks map. Markdown is derived. Passive viewers and client Markdown-only updates cannot overwrite content.
  • One durable writer: a worker owns SQLite and serial command execution. HTTP adapters and Hocuspocus rooms are consumers/protocol mirrors. A process ownership lock refuses a second owner of the same database.
  • One commit boundary: role, access generation and explicit freshness are checked before mutation. A disposable candidate is validated; binary state, projection, revision, event, tombstone and idempotency receipt commit together. SQLite uses WAL and synchronous=FULL.
  • Durability precedes acknowledgement and publication: no delayed onStoreDocument writer, dirty-room snapshot, route-time reseeding, repair loop, distributed lease selection, or global quarantine breaker remains in the default app.
  • Generation fences: access changes and owner restores invalidate tickets and evict old connections. Restores and clones use fresh CRDT identities. Client durable storage is partitioned by document identity and generation; old buffers remain available locally but cannot replay into a new generation.
  • Bounded work: 128 pending commands, 16 per document, 128 rooms, 1 MiB update frames, 2 MiB documents, 6 MiB CRDT state, 20,000 nodes, 500-character titles, a 200-entry metadata-only library, and a 15-second deadline. Parsing/persistence run off the HTTP event loop. Homepage, favicon and health do not await document commands.
  • Explicit recovery: corrupt binary yields the last atomically stored projection, read-only, without changing the record. An owner restores a recorded valid binary revision. Nothing guesses missing document text from a stale projection or mark quote.

The auth adapter exchanges OAuth identity separately, then uses locally persisted opaque account sessions; document refresh does not call the identity provider. Library entries are a bounded metadata read view over owned/visited documents, with no full document/mark payloads returned to the HTTP serializer. Notes & Pins is one normal live document per account, rather than an automatic competing library synchronization writer. Disconnect now destroys the client's private provider transport as well as the document provider, eliminating leaked reconnect timers.

Shutdown keeps signal handlers installed during the drain: repeated SIGTERM/Ctrl-C is idempotent rather than terminating mid-drain. The test sends repeated signals and asserts clean exit.

The shared image parser now normalizes omitted mdast title/alt values to strings. Without that correction, the installed Milkdown schema silently dropped title-less images. The new integrated rich-content test caught this.

Verification commands and results

Repeated development runs are included below by their distinct command. Failures are retained here rather than presented as successful verification. Final required gates include the new default-runtime integration suite; no workflow file changed. Final outputs and exit codes are saved in docs/verification/commands.json and the accompanying *-output.txt files. The test-author skill used was .agents/skills/proof-collab-test-author/SKILL.md, using its local harness/authority assertions; no external docs gate was run.

Command Result
npm run build PASS, including Vite editor output and build:mcp (tsc -p tsconfig.mcp.json plus local bundling). Repeated after client/parser changes; final exit 0. Vite reports its existing large-chunk warning.
npm run test:ci PASS, final exit 0; original nine CI targets plus the new running-app suite.
npm run test:pr:collab PASS, final exit 0; simulations/contracts/history anchors, both retained shutdown regressions, and the new running-app suite.
npm run typecheck:core PASS, strict core typecheck, exit 0.
npx tsc -p tsconfig.core.json Final PASS. Development runs initially found type errors, later a missing block-edit helper; all corrected.
npx tsc FAIL, exit 2, in both untouched frozen archive and current checkout. Frozen: 1,127 diagnostics; current: 1,109. No file/error-code category increased. Existing rootDir, unused, library and old application typing problems remain. The root tsconfig was not narrowed to hide them. See typecheck-comparison.json.
npm run test:core PASS, exit 0, repeatedly against real default-server child processes and local clients. The final version includes rich content, commenter PUT and pending-mark restore.
npx tsx src/tests/document-core-integration.test.ts Final PASS. Development failures exposed missing legacy edit-operation adaptation, shutdown exit reporting, domain strings passed as numeric WebSocket close codes, anonymous identity authorization, transport teardown and title-less image loss. Those defects were corrected and the complete suite rerun. One successful early run printed PASS but retained a leaked client transport interval; it was interrupted (exit 130), then the cleanup defect was fixed and subsequent runs exited normally.
npm test Initial development runs FAIL at the untouched legacy escaped-underscore rehydration assertion. Final offline aggregate invocation below disables legacy remote telemetry.
PROOF_ENABLE_TELEMETRY=0 npm test PASS, final exit 0, including retained default/server-route tests and the running new core. Earlier runs failed at the frozen legacy rehydration assertion; a subsequent full run reached the new rich-image assertion, exposing the image loss fixed above. Another aggregate passed its retained targets but exposed intermittent signal termination during the final integration shutdown; persistent signal handlers and a repeated-signal regression corrected it.
npm run test:collab:stability-followups PASS, final exit 0, including the running core suite.
PROOF_ENABLE_TELEMETRY=0 npm run test:collab:stability-followups PASS, exit 0, after explicit architecture replacements; retains scoped quarantine, global admission, large-doc, repair-loop and compaction historical tests and adds the running new core suite.
npm run test:collab:contracts PASS as executed inside both collaboration PR runs and the full/server-route aggregate: Yjs simulation, real Hocuspocus contracts and history-anchor contracts.
npm run test:server-routes-share PASS, executed as part of the final npm test aggregate (exit 0), including its new core suite.
npx tsx src/tests/marks.test.ts PASS, after the shared image normalization.
npx tsx src/tests/milkdown-headless-serializer-roundtrip.test.ts PASS, before and after shared parser changes.
PROOF_ENABLE_TELEMETRY=0 npx tsx src/tests/milkdown-collab-runtime.test.ts PASS, maintained source assertion now requires same-generation replay. Initial old assertion failed.
PROOF_ENABLE_TELEMETRY=0 npx tsx src/tests/collab-regression-guards-round2.test.ts PASS, stale assertion corrected to the frozen editor's actual retry default of 240. Initial assertion expected 12.
PROOF_ENABLE_TELEMETRY=0 npx tsx src/tests/mutation-contract-stage-and-ws.test.ts Before the named retirement: FAIL, 22 passed and the obsolete hosted-force/live-lease-disconnect case failed. Final aggregate executes the remaining 22 cases and reports the retirement.
npx tsx src/tests/proof-mark-rehydration.test.ts FAIL, repeatedly at escaped-underscore legacy repair, also in the untouched local frozen archive. Retired from aggregates with named replacement coverage; no legacy repair code fix was retained.
node .tmp/run-retained-tests.mjs Initial exhaustive audit: FAIL, 133/145 distinct existing local targets passed, 12 failed. It executes each target with node --import tsx <test> and records every result in retained-tests.json. Nine obsolete files and one obsolete case are explicitly replaced; two source assertions were maintained. The final aggregate validates the retained default/server-route targets.
npx tsx .tmp/rich-probe.ts Exit 0, but diagnostic output showed the shared parser dropping an image after an attribute validation error. This prompted the fix; the integrated rich-content regression subsequently passed.
npx tsx -e worker/store create-read-close probe PASS, real worker and isolated .tmp/core-probe.sqlite.
npx tsx -e headless parser import probe FAIL: the eval's CommonJS form cannot import the installed ESM-only Milkdown package. File-based tsx probes and the default server work; no dependency was installed or changed.
node --import tsx server/bootstrap.ts Actual default server launched by the integration suite with isolated DB/ports; graceful shutdown exit 0 and deliberate SIGKILL/restart verified.
node --import tsx scripts/mock-every-oauth.ts Local OAuth fixture launched, exercised, terminated and awaited by the integration suite.
APPSIGNAL_LOG_PATH=$TMPDIR npm run serve (with an isolated PROOF_CORE_DB_PATH for the preview run) Launched successfully, /health ready, homepage 200, document creation 201 and built editor 200. Preview was stopped; JSON shutdown completion observed and command exited 0.
node .tmp/manual-exercise.mjs PASS, exit 0, actual preview entry/API/editor probes; manual-app-observations.json.

Telemetry was disabled for the final legacy aggregate so its retained PostHog adapter would make no external request. Earlier retained legacy tests attempted their default telemetry calls; the fixture blocked them and they provided no external evidence. The new core does not initialize remote telemetry.

Capability inventory and regression coverage

Capability Delivered behavior / status Evidence and limits
Existing web editor and default APIs Kept; /d/:slug serves built editor/config and its ShareClient/CollabClient uses the new core. Actual default-server HTML/assets observed; real CollabClient instantiated against it. Full browser DOM interaction unavailable.
Rich Markdown Kept: frontmatter, formatting, links, images, task lists, tables, code and quotes; canonical serialization may normalize source formatting. Real create, provider hydration and block snapshots; retained parser/serializer tests. Image loss fixed in shared format plugin.
Multi-client collaboration Kept; concurrent real providers converge to canonical HTTP content. Three-client join, concurrent edits, awareness, HTTP edits published to other providers.
Passive join / duplicated documents or content Corrected: no passive canonical seed/write; fragment-derived projection; same-key create retry returns the same document. Viewer revision stability, Markdown-only correction, repeated creation, pathological whole-document replay rejection, clone identity isolation. Optional idempotency keys are needed for retry-safe creation.
Human and agent content edits Kept: fresh content PUT/rewrite, legacy edit operations and structural block-ref edits use the same command authority. Live providers plus agent replacement, block insertion, bridge rewrite, stale base 409, durable events. Some operation variants are implementation-covered but not individually exercised.
Comments, replies, resolution Kept through HTTP and existing editor marks PUT; anchors belong to structured content. Commenter HTTP comment/reply/resolve/unresolve and commenter metadata PUT; attempts to change suggestions/authorship denied. Comment focus/draft UI remains covered by retained source tests, not browser execution.
Suggestions / provenance Kept: API proposals, inline insertion rejection, accept/reject, long quotes, split ranges/nested comments, authored marks and finalized-ID fencing. Integrated mark regressions plus retained schema/mark tests. Explicit restore intentionally reinstates historical pending marks in a fresh generation.
Share roles and ownership Kept (including JSON or text/markdown share creation and atomic role-specific creation links): slug capability editing, explicit viewer/commenter/editor links, owner secret/account ownership; fresh expiring collab tickets. Viewer edits denied, commenter boundaries, anonymous owner-control denial, account-owned read/write, old ticket rejection.
Lifecycle and title Kept: title, pause/resume, rotate/revoke/delete and owner quarantine. Title/pause/resume/rotate/delete exercised. Revoke and explicit owner quarantine share implemented epoch/eviction paths but were not separately exercised.
Offline reconnect and durable buffers Changed: bounded same-generation deltas; old identity/generation buffers cannot replay. Actual CollabClient hard reconnect and Storage reload, provider offline replay, restart and duplicate-update idempotency. Old buffer bytes remain locally recoverable; no recovery UI was added.
Persistence and shutdown Corrected: accepted edits are already durable, graceful drain and abrupt-loss recovery. Graceful restart equality, SIGKILL after provider ack, real SQLite trigger failure with unchanged binary/revision/events and no broadcast, successful reconnect retry.
Read-only recovery / owner restore Changed: side-effect-free readable last commit; explicit known-binary-history restore, fresh CRDT generation. All durable editing/commenting disabled when binary is corrupt. Actual stopped-server corruption injection, read/context capabilities, record unchanged after reads, failed PUT, history repair and new provider admission; pending marks restored. No Markdown/JSON auto-repair.
Canonical clone Kept; fresh document identity and CRDT client IDs, marks preserved, transient presence excluded. Provider state-vector identity non-overlap and clone-only edit isolation.
Events and presence Events kept atomically with commands; explicit agent presence moved to ephemeral /ws events consumed by existing editor fallback. WebSocket updates/presence and automatic explicit-agent request presence, pending-event polling/ack; no content revision change for presence. Cursor hints and inferred user-agent identities removed.
Every sign-in Implemented separate adapter; persistent one-hour local account session. Repository mock OAuth authorize/callback/poll, secret poll check, owner session after restart. External provider not contacted; refresh tokens/long-running provider refresh are missing.
Library / Notes & Pins Changed: account-scoped read view plus one normal live notes document; no sync writer. Owned documents and notes listed, repeated notes creation returns same slug, account authorization and restart. Old live library projection/pinning automation is not retained.
Bug reports Local storage kept; report endpoints return local receipt. API exercised; no external issue submission.
Native app / MCP / CLI Existing client/source and MCP build retained; normal document APIs available to them. MCP TypeScript build and retained bridge unit tests pass. Swift/macOS host, packaged native app, MCP process end-to-end and every command variant were not exercised. Browser-only bridge selectors/UI execution endpoints are missing (unknown APIs return 501).
Social previews / cloud operations Missing from the new runtime: dynamic social image rendering, remote analytics/AppSignal integration, cloud volume repair tooling and production operational dashboards. Static homepage/editor assets remain. Legacy operator tools do not operate the new store; structured lifecycle/request logs and local health counters replace runtime telemetry for this fixture.

What was directly observed

core-runtime-observations.json records successful assertions against launched server/bootstrap.ts processes. These used loopback HTTP, real Hocuspocus WebSockets/Yjs clients, the real browser CollabClient with local Storage adapters, SQLite persistence, and the repository's loopback OAuth fixture. Tests actually ran; they are not merely written specifications.

The suite observed content and marks on HTTP, live providers and durable storage surfaces; concurrent human/agent propagation; healthy new-document admission; generation eviction; clone independence; graceful/abrupt restart; denied viewer/commenter violations; and atomic persistence-failure behavior. Under 80 concurrent stale requests and a pathological document update, homepage, health and favicon completed under the asserted two-second bound and another document remained editable. This is a bounded local regression, not evidence of production-scale throughput or a reproduction of the 40-minute outage.

The literal npm run serve preview returned ready health, served the homepage and injected editor configuration/built assets, and created/read a new-core document. It was shut down and awaited. Chromium launch failed because its executable is absent; nothing was downloaded. Consequently no rendered-editor screenshot, keyboard/focus/selection/mobile interaction, real browser refresh, or native host execution is claimed. The suite's optional browser path will exercise the built editor when an executable is available.

Explicit test retirements

The complete named mapping is docs/verification/test-replacements.md. Nine legacy authority/browser-runtime files were removed from aggregate invocations, and one named hosted-force case retired while its other 22 cases remain. Each has new default-runtime coverage or a stated browser/UI gap. Their source files remain. Other aggregate tests were kept. test inherits the new suite through test:server-routes-share (avoiding a duplicate integration invocation). Those scripts and test:collab:stability-followups were updated because those invocations exercised replaced authority paths; test:ci and test:pr:collab now also run the new core. .github/workflows is unchanged.

Remaining limitations

  • This is a single-host authority. The lock is a process ownership check, not a distributed consensus or storage lease. Simultaneous-start races were not stress-tested. A crash during the short .claim acquisition can require manual claim-file cleanup; PID reuse can fail closed.
  • One worker serializes documents; slow work can delay document APIs. A 15-second command deadline terminates the executor and requires process restart. Public routes remain separate, but there is no supervisor or automatic worker recovery. No production soak, distributed failure, full-size saturation or long-haul performance measurement was possible.
  • History retains the current and most recent 100 revisions. CRDT state exceeding 6 MiB is rejected until an explicit restore/clone resets its identity. Event, idempotency-receipt, report and document retention/backup policies need operational work.
  • API source formatting is canonicalized. Legacy truncated-wrapper reconstruction, ambiguous-anchor guessing and JSON reseeding were intentionally removed. Replacing text can leave an unrelated comment as metadata when its old text disappears; a complete orphan-anchor UX is not implemented.
  • Browser and native UI coverage, external OAuth behavior, provider refresh, cursor hints, richer library/pinning behavior, browser-only bridge operations, social rendering and remote operations integrations remain the inventory gaps above. This is an integrated local app, not a claim of production feature parity or deployment readiness.
  • The broad npx tsc remains red on pre-existing repository typing problems; the new strict core and MCP build pass. No external documentation/promotion review gate was performed: the Benchmark Fixture Override prohibits those lookups and remote operations.
  • Final tests own and await their child processes. An early failed test left an ownership file whose PID cannot be verified/signalled from a later sandbox tool call (EPERM); its original command is completed. Final preview and integration shutdowns completed normally. The session sandbox stops any remaining earlier child at session end.
